Nuclear Subs Collide

First satellites collided now subs collided. My question is how the F did this happen? Don't these large machines have equipment to know what is nearby. If not, I think they have a major design flaw.

Both Saunders and Willett said submarines don't always turn on their sonar systems, or make their presence obvious.

Anybody could enlighten me as to why on earth they wouldnt turn on their sonar systems? Would they then become easily detectable themselves?

Sonar is an acoustic system. A sub moving around with its sonar engaged is broadcasting to its enemies: Here I am!
